New/Transferring, Withdrawing Student, or First Device
Issue with TEAMS
Complete this form if you are a current student but has never received a device or if you are enrolling in Manassas for the first time. Also, complete this form if you are withdrawing from Manassas.
Complete this form if you are experiencing issues with their device. It could be damaged or not working.
Complete this form if you have lost your device or if your device was stolen. If it was stolen, you will need to file a police report and provide that number on this form.
This form is for students who are having trouble with TEAMS (passwords, video/audio, calendar, etc).
